General Conditions to Our Gift Vouchers
1. The gift voucher can be issued for procedures and additional services (a), services of the spa restaurants (b), and for a stay in the spa facilities (c):
a. Gift Vouchers for procedures and additional services can be bought at reception desks of individual spa houses. The holder of this voucher may withdraw services continuously; with each case of withdrawal being recorded on the other side of the voucher and confirmed by the stamp and signature of a responsible employee of the spa.
b. Gift vouchers for services of restaurant facilities can be bought at reception desks of individual spa houses. It is possible to use the voucher only once in any of the spa restaurants.
c. Gift Vouchers for the stay in our spa must be ordered in the spa business department.
2. The gift voucher for value can be purchased at reception desks of individual spa houses; for the values from CZK 50.00 up to CZK 5,000.00 –you can order the value which is always a multiple of 50. It is possible to withdraw services from the voucher for all products and services rendered by Lázně Teplice v Čechách, Capital Company limited by shares, under the conditions referred to in Item 1.
3. Gift Vouchers are valid only if they bear a serial number, a bar code, the subject of withdrawal and the seal and signature of a responsible spa employee.
4. The Gift Voucher is valid until 31st December 2009, provided it is not stated otherwise on this voucher.
5. The Gift Voucher of Lazně Teplice a.s. is issued on the basis of our acceptance of your payment, which can be effected in the following forms: cash, by means of the bank transfer or by the money order.
6. The withdrawal of services in compliance with the specification of the Gift Voucher must be booked well ahead; with the spa stay 30 days beforehand, with services at least 10 days ahead.
7. It is always necessary to submit your Gift Voucher before you start withdrawing any of the services.
